What Connexions Trafford can do for LDD young people in Trafford
Connexions Advisers work with young people between the ages of 13 to 19.
If you have a learning difficulty or disability Connexions can offer support until your 25th birthday.
Here are some ways your Connexions Adviser can help:
- Give information about choices
- Help you plan what you want to do in the future
- Talk about any problems that you have and support you to get help
- Go to your review meetings
- Talk to your family or carers about your options
It can be a worrying time getting ready to leave school and move into adulthood (known as transition).
The Easy Read Guide to Transition has been designed to help explain the transition process and answer some of your questions.
The guide gives a brief introduction to transition and the many opportunities after 16.
